Consumer Directed Choices (CDChoices) is proud to announce its partnership with Lizzy Care through an agreement with GuideWay Health, PLLC, the Medicare Guiding an Improved Dementia Experience (GUIDE) practice entity. Working together, CDChoices will serve as the Respite Provider, delivering flexible, person-centered respite care services to families affected by dementia across the Bronx, Brooklyn, Manhattan, Queens, Staten Island, Orange, Rockland, and Westchester counties.
The GUIDE Program is an innovative Medicare initiative designed to enhance care coordination, education, and access for individuals living with dementia and their caregivers. By connecting families to community-based supports, the program helps them navigate the challenges of dementia care while promoting independence and dignity at home. GUIDE also focuses on improving caregiver well-being by offering respite opportunities to reduce stress and prevent burnout. Medicare recipients may be eligible to participate in the GUIDE program at no cost, making these vital services accessible to more families in need.
