Consumer Directed Choices (CDChoices) is proud to announce a new partnership with Hudson Valley Care Coalition (HVCC), the designated Social Care Lead Entity and Social Care Network (SCN) for the region. This partnership marks a significant milestone as CDChoices begins serving individuals through the New York State Medicaid 1115 Waiver program.

Through this partnership, CDChoices will serve as both Screener and Social Care Navigator, assisting Medicaid-eligible individuals identify and address health-related social needs that impact their health, stability, and independence. The 1115 Waiver program supports individuals of all ages experiencing challenges such as housing instability, food insecurity, transportation barriers, or caregiving difficulties. Its goal is to ensure that every Medicaid-eligible individual receives an annual social needs screening, and that those who screen positive are connected with navigation services and support eligibility assessments for additional resources.

Working alongside HVCC, CDChoices will deliver these essential services throughout Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, and Westchester counties. By collaborating with HVCC and local community organizations, CDChoices will play a key role in coordinating non-medical interventions designed to improve health outcomes, enhance stability, and promote independent living for Medicaid members.

HVCC, a respected nonprofit organization, has long been a trusted connector for clinical, behavioral, and social care by bringing together community organizations and resources to address the factors that shape health and well-being throughout the Hudson Valley. By leveraging HVCC’s established network and deep community relationships, this partnership will enable CDChoices to extend its person-centered approach and reach more individuals.

Together, HVCC and CDChoices are well positioned to advance equity, expand access, and improve outcomes, making a meaningful difference in the lives of Medicaid members and their families throughout the Hudson Valley.
