Consumer Directed Choices (CDChoices) is proud to announce a strategic partnership with Eli Health through an agreement with Lifetime Redesign OT, PLLC, the designated practice entity for Medicare’s Guiding an Improved Dementia Experience (GUIDE) Model. Through this collaboration, CDChoices will serve as the Respite Provider, supporting comprehensive assessments and delivering flexible, Caregiver-Directed respite care services to families affected by dementia in New York State and Connecticut.
The GUIDE Model is a Medicare initiative designed to enhance care coordination and improve quality of life for individuals living with dementia, while strengthening support for family caregivers. The program provides structured care management, caregiver education and training, access to ongoing support, and respite services at no cost to eligible Medicare beneficiaries. By connecting families to coordinated clinical and community-based services, the GUIDE Model helps reduce caregiver strain and improve long-term care outcomes.
As a nonprofit organization founded in 1997, CDChoices brings over two decades of experience advancing independence and self-direction for older adults and individuals with disabilities. The organization is a recognized leader in self-directed home care services across New York State. Through its caregiver-directed model, families have the freedom to recruit, hire, train, and manage their own caregivers, whether that is a trusted family member, a friend, or someone identified through CDChoices’ Workforce Recruitment Portal. This approach preserves dignity, strengthens continuity of care, and ensures services remain centered around each individual’s needs.
Eli Health is an approved national Participant in the GUIDE Model and specializes in dementia care navigation and caregiver support services. As part of the GUIDE Program, Eli Health provides individuals and families with personalized care coordination, education, and access to respite resources, ensuring they are not navigating dementia care alone.
Together, CDChoices and Eli Health are expanding access to person-centered dementia support across New York State and Connecticut. By integrating comprehensive assessments, care navigation, and flexible caregiver-directed respite services, this partnership addresses a critical gap in dementia care by providing families with practical, sustainable support.
