Ensure a Smooth Transition with Consumer Directed Choices
As of January 6, 2025, New York State has transitioned to a Single Fiscal Intermediary (SFI) for the Consumer Directed Personal Assistance Program (CDPAP). Consumer Directed Choices (CDChoices) has been selected as an approved CDPAP facilitator, authorized to support you and your personal assistants (PAs) during this critical transition.
To avoid any disruption in care, it is crucial that all consumers and PAs complete the necessary onboarding paperwork by March 28, 2025. CDChoices is here to help. Consumers
- Are required to ensure all required paperwork is completed by March 28, 2025. Additionally, your PA(s) must complete all necessary training and paperwork by this deadline.
Personal Assistants
- Need to communicate with your consumer regarding the transition and begin the registration process.
- Must complete and sign all required forms and documents.
- Are required to fulfill all mandatory training requirements.

Complete an orientation and all required onboarding documents for both you and your PAs and submit everything by March 28.
