Empowering Independence Across Connecticut
CDChoices serves as Connecticut’s statewide provider for caregiver-directed Respite Care Programs, continuing our 25-year mission to support independence, dignity, and choice in care.
Through partnerships with the Eastern Connecticut Area Agency on Aging (ECAAA), North Central Area Agency on Aging (NCAAA), and Western Connecticut Area Agency on Aging (WCAAA), CDChoices helps participants and their families access support and relief through the Connecticut Statewide Respite Care Program and the National Family Caregiver Support Program.
National Family Caregiver Support Program (NFCSP)
The National Family Caregiver Support Program (NFCSP), funded by the Connecticut Department on Aging, supports unpaid caregivers who provide care to:
- Individuals age 60 or older
- Individuals with Alzheimer’s disease or related disorders (regardless of age)
- Caregivers age 60 or older caring for an adult child with a disability
- Grandparents and relatives age 55 and older raising grandchildren
Eligible care recipients must require assistance with at least two Activities of Daily Living (ADLs), such as bathing, dressing, toileting, eating, or transferring/walking.
Connecticut Statewide Respite Care Program (CSRCP)
Similarly, the Connecticut Statewide Respite Care Program (CSRCP) is funded by the Connecticut Department on Aging and administered in partnership with the regional AAAs and the Connecticut Chapter of the Alzheimer’s Association. This program focuses on families caring for individuals diagnosed with Alzheimer’s disease or other irreversible dementias, including multi-infarct dementia, Parkinson’s disease, Lewy Body dementia, Huntington’s disease, Normal Pressure Hydrocephalus, and Pick’s disease.
Through CSRCP, families receive assessments and individualized care plans, with access to a range of services such as adult day care, cognitive fitness training, home health aide services, homemaker and companion services, personal care assistance, skilled nursing, and short-term nursing care. The program is designed to reduce caregiver stress while enhancing quality of life for both the caregiver and the care recipient.
Caregiver-Directed Respite Services
By partnering with CDChoices, caregivers can direct their own respite services, hiring and managing trusted providers to deliver personalized, compassionate support. This self-directed model ensures care is tailored to the unique needs of your loved one, giving you greater control and peace of mind.
In addition to respite care, CDChoices connects families with essential resources, including caregiver education, emotional support groups, and individualized care planning assistance. These services empower families to navigate the challenges of caregiving with confidence, ensuring both caregivers and their loved ones receive the support they need to thrive.
