Connecticut Respite Care Program

Important Transition Information

If you are currently enrolled with Allied Community Resources, your services will soon transition to Consumer Directed Choices (CDChoices) on December 22, 2025.

By November 30, 2025, a CDChoices team member will contact you by phone or email to introduce themselves and schedule a brief orientation, by phone or video, at a time that works best for you. During this orientation, we will:

  • Answer any questions you have about the transition

  • Review and assist you with your transition paperwork (including your Caregiver/Employer Agreement and your worker’s hiring and tax forms)

  • Walk you through how to record and approve hours

  • Show you where to find help and ongoing support

🕒 Important: All required paperwork must be submitted before your first scheduled service date with CDChoices. This ensures your worker is cleared to begin on time and prevents any interruptions in care.

The transition of services will begin at 12:01 a.m. on Monday, December 22, 2025.

Empowering Independence Across Connecticut

CDChoices serves as Connecticut’s statewide provider for self-directed Respite Care Programs, continuing our 25-year mission to support independence, dignity, and choice in care.

Through partnerships with the Eastern Connecticut Area Agency on Aging (ECAAA), North Central Area Agency on Aging (NCAAA), and Western Connecticut Area Agency on Aging (WCAAA), CDChoices helps participants and their families access support and relief through the Connecticut Statewide Respite Care Program.

Connecticut Statewide Respite Care Program

Supporting caregivers of individuals with Alzheimer’s disease or related dementias, this program aims to reduce caregiver stress and improve quality of life for both the caregiver and the person receiving care.

By working with CDChoices, caregivers can direct their own respite services, hiring and managing someone they trust to provide personalized, compassionate support. This model ensures care is tailored to your loved one’s unique situation while giving you greater control and peace of mind.

In addition to respite care, the program connects families with essential resources such as caregiver education, emotional support groups, and individualized care planning assistance. These services help families navigate the challenges of caregiving more confidently, ensuring that both caregivers and their loved ones receive the support they need to thrive.
