A Four Step Guide

RECEIVE YOUR EVALUATION
First, a practitioner through the Department of Health (DOH) and Maximus Health Services, Inc., contracted under the New York Independent Assessor (NYIA) will complete a home care evaluation and issue a practitioner’s order required to authorize services. CDChoices does NOT determine eligibility for Consumer Directed Personal Assistance (CDPA).

DETERMINE YOUR ELIGIBILITY
Your Local Department of Social Services or Managed Care Plan will determine if you are self-directing and how many hours of care you are eligible to receive. If you are not self-directing, a designated representative can direct your care.

CHOOSE YOUR ASSISTANTS
