Guiding an Improved Dementia Experience (GUIDE) Program

The Medicare Guiding an Improved Dementia Experience (GUIDE) Model is a national initiative designed to improve care for people living with Alzheimer’s disease and other dementias while providing meaningful support for family caregivers.

Through the GUIDE Model, eligible Medicare beneficiaries can access coordinated dementia care services, caregiver education, and respite support to help individuals remain safely in their homes and communities.

Consumer Directed Choices (CDChoices) supports the GUIDE Model by providing Caregiver-Directed Respite services, giving families the flexibility to choose who provides care while they take needed breaks from caregiving.

What CDChoices Provides Through GUIDE

CDChoices delivers Caregiver-Directed in-home respite services to support family caregivers participating in the Medicare GUIDE Model.

Our consumer-directed approach allows families to choose a caregiver they know and trust while we handle the administrative responsibilities required to support employment and payroll.

Through CDChoices, caregivers can:

  • Select and hire a caregiver they trust, such as a family member, friend, or community member
  • Use our Workforce Recruitment Portal to find potential caregivers
  • Determine when and how respite care is provided
  • Receive administrative and payroll support for their respite worker

Eligible caregivers may receive $2,563 per year in Medicare-funded respite services through the GUIDE Model (July 1 – June 30).

About the Medicare GUIDE Program

The Medicare GUIDE Model is an innovative program developed by the Centers for Medicare & Medicaid Services (CMS) to improve dementia care across the United States. The program focuses on helping individuals living with dementia remain in their homes and communities while providing caregivers with the support they need.

Participants receive a comprehensive set of dementia care services that may include:

Coordinated Care Management

A dedicated dementia care team that helps coordinate healthcare services and supports.

Personalized Care Planning

Individualized care plans developed based on each person’s needs, preferences, and health goals.

Caregiver Education and Coaching

Training and resources to help caregivers better understand dementia symptoms, safety strategies, and behavioral changes.

Respite Services

Eligible caregivers may receive financial support for respite care so they can take time to rest and recharge.

Who Is Eligible for the GUIDE Program?

To participate in the GUIDE Model, individuals must generally meet the following criteria:

  • Have a confirmed diagnosis of dementia from a participating GUIDE clinician
  • Be enrolled in Medicare Parts A and B, with Medicare as their primary insurance
  • Not be enrolled in a Medicare Advantage plan, Special Needs Plan (SNP), or PACE program
  • Not have elected the Medicare hospice benefit
  • Not reside in a long-term nursing home

Eligibility and enrollment are coordinated through participating GUIDE care providers.

The program provides comprehensive dementia support through personalized care coordination and transitional care management, caregiver education and support, and 24/7 access to services. Participants receive individualized care planning, comprehensive assessments, and home visits, as well as medication management and reconciliation. The program also offers ongoing monitoring, coordination of social services, and referral to additional resources, with a focus on supporting both the person with dementia and their caregivers. CDChoices specifically helps families through in-home, caregiver-directed respite services, allowing caregivers to take needed breaks while ensuring safe, compassionate care at home.

Eligible Caregivers receive $2,563 a year in GUIDE respite services from July 1-June 30. This gives Caregivers the choice to hire a trusted individual such as another family member or a friend, to provide temporary in-home care, allowing them to take breaks, attend appointments, or rest, while ensuring the person with dementia receives safe, compassionate care. CDChoices handles the hiring process, including caregiver paperwork, weekly payroll, and benefits such as paid time off and workers’ compensation, so families can focus on care rather than administrative tasks.

 

For eligible Medicare Parts A and B beneficiaries, the GUIDE Program is free. Services like dementia care coordination, caregiver support, and in-home respite are covered at no additional cost. Costs may apply only for services outside the program or not covered by Medicare.
