Consumer Directed Choices (CDChoices) supports family caregivers across New York by administering consumer-directed respite services that allow caregivers to choose someone they trust to provide care while they take a needed break. These services help caregivers maintain their own health and well-being while continuing to support their loved ones at home.

Our respite programs support caregivers in two situations: those caring for individuals living with dementia and those caring for family members or other unpaid caregivers assisting adults who are frail or living with disabilities, regardless of whether dementia is present.

CDChoices provides non-Medicaid consumer-directed respite services through the Alzheimer’s Disease Regional Caregiver Support Initiative, a grant-funded program administered by the New York State Department of Health. CDChoices began administering consumer-directed respite services since 2019.
