CDChoices Response to COVID-19

In an effort to do our part to help stop the spread of the novel coronavirus (COVID-19), our office is temporarily closed. Staff will be working, but we will not be open to the public.

CDChoices is temporarily suspending in-person meetings with our Consumers, Designated Representatives and their Personal Assistants. If you need assistance, have questions, or would like to discuss an issue or concern, our staff are available by phone or email. We can also arrange for web-video conversations as well, if you wish.

In addition, during this public health crisis, we ask all of our Consumers and Designated Representatives to use email, fax or mail to promptly submit important paperwork or time sheets, instead of coming to the office. If you do need to hand-deliver paperwork or time sheets, we have drop boxes available outside our office and at the back of our building so that you do not have to come inside our office.

This was a difficult decision to make. We truly value the face-to-face interactions we have with all of you because we know it helps us best connect and support you as your manage your care. But with the spread of this new coronavirus and the danger it poses to so many we serve daily, we felt it was best to stop in-person meetings until this public health crisis is over.
